Hello families,

If your student is entering Pre-K, Kindergarten, 2nd Grade, 6th Grade, 9th Grade, or 12th Grade there are Required Health Examinations for those students.  Below you will find what is required by grade. The State of Illinois requires health examinations for the 2023-2024 school year be submitted by October 15, 2023. Failure to comply will result in your student being excluded from school.

Pre-K Students:

  • IL Physical Exam Parents Please complete the health history portion, sign and date

  • Complete updated Immunization record signed by a physician

Kindergarten Students (K)

  • IL Physical Exam Parents please complete the health history portion, sign and date

  • Complete updated Immunization record signed by a physician

  • Dental Exam on Illinois Form dated between November 2022 - May 15, 2024

  • Eye Exam on Illinois Form completed by Optometrist within 1 year of enrollment due by October 15, 2023

Second Grade Students (2nd)

  • Dental Exam on Illinois Form dated between November 2022 - May 15, 2024

Sixth Grade Students (6th)

  • IL Physical Exam Parents Please complete the health history portion, sign and date

  • Complete updated Immunization record signed by a physician

  • Dental Exam on Illinois Form dated between November 2022 - May 15, 2024

Ninth Grade Students (9th)

  • IL Physical Exam Parents Please complete the health history portion, sign and date

  • Complete updated Immunization record signed by a physician

  • Dental Exam on Illinois Form dated between November 2022 - May 15, 2024

Twelfth Grade Students (12th)

  • Complete Immunization Record signed by a physician, showing second dose of Meningococcal vaccine

Religious/Medical Exemptions:  If your child does not receive immunizations for religious reasons, a parent and health care provider must complete the “Illinois Certificate of Religious Exemption to Required Immunizations and/or Examinations” form prior to the first day of school.  This form may not be used for personal or philosophical reasons.  Illinois does not allow for such exemptions. A new exemption form must be completed each year your student is required to have updated vaccinations.

Sports Physicals:  If your child will be participating in any sport, they must have a sports physical on file prior to the season’s first practice.  Incoming 6th and 9th grade may use an IL school physical in lieu of a sports physical. However a sports physical CAN NOT be accepted as a proof of a physical examination for mandated physicals. Sports physicals are valid for 395 calendar days from the date the physical was issued.

New Students

New students enrolling in a Illinois school for the first time will be required to obtain & submit the following within 30 days of enrollment:

  • Physical examination on the IL physical form

  • Up-to-date immunization record regardless of grade

  • Eye Exam on IL form

Medications/Forms

If your child requires medication during the school day, a new medication authorization form is needed each school year.  If your childs requires Emergency medications to be on file at school, a new medication authorization form for each medication as well as an emergency action plan must be completed and turned in to the school nurse. No medications will be given to students without proper authorization from a health care provider, this includes over the counter medications.  Medications, with the exception of inhalers, will NOT be sent home with students. Parents must make arrangements to pick up medication from school. I strongly discourage parents sending medication to school with students.

Required Health Examinations are due October 15, 2023. To prevent students from being excluded from school, please submit this information to School Nurse, Sarah Bell, bells@husd4.org or fax to 309-857-0157. All health forms can be found at www.husd4.org - Families/Health Services.
